Maximum Value: $4987. No cash value. $4987 to be applied as a credit balance to certificate holder’s account.
May be applied to one or more adolescent or adult orthodontic treatments, up to a maximum of $4987.
Includes orthodontic records (x-rays and photos).
Amount may be applied to other therapy (Spark- which is like Invisalign, etc.)
The certificate cannot be used with insurance.
Original certificate must be brought at the time of treatment.
Also, there is not a charge for a New Patient appointment. And that is across the board for all of our patients.
Additional Information
PLEASE NOTE: Winners will pick up their items in the school cafeteria during Spring Festival on April 24th from 5pm-8pm. Winning Items that can be emailed, will be emailed prior to the Spring Festival to the email address of the highest bidder. If you are a friend/relative (of a Livsey family) who lives outside of Tucker, you will need to make plans with that friend/relative to pick up your item and deliver it to you. We will not mail or ship any auction items. Remember this auction is set up by volunteers.
