One full comprehensive eye exam with Dr. Lindsay Michaud at the Kittery Optometric Associates office on 99 US Route 1 Byp, Ste A, Kittery ME 03904. An option for a contact lens evaluation and prescription for a current contact lens wearer (no new fits). This does not include follow up care or imaging (will be offered a second follow-up visit if recommended) Value: Full comprehensive eye exam for new patient ($227) with refraction and glasses prescription ($60). Option of a contact lens evaluation, trials, and a prescription for current contact lens wearer ($55). Winner can contact the office to schedule their eye exam between 8-5 M-F, or can contact me directly by phone or email if they have questions or need Saturday availability. No expiration on this service, for example if they are due for their yearly visit in 8 months then we can use it then. They would come into the office for their eye exam like normal. Winner will receive additional directions. No imagining included, no glasses and contact lenses provided besides contact lens trials. I may recommend follow up care, which is not included.
